Floor Medix does not store full credit card numbers on this website. By submitting payment, you authorize Floor Medix and its payment processor to charge the payment method provided for the invoice amount shown.

Payments made toward issued invoices are final unless otherwise stated in your written service agreement or authorized by Floor Medix in writing. Refunds, credits, or adjustments may be reviewed on a case-by-case basis at the sole discretion of Floor Medix.
